Update:

It also happens even when you're not using HP cut/drain. If your opponent manages to cast anything before your Susanoo procs, that will also trigger the mirror bug.

As what I've observed, this happens when you inflict 2 waves of dmg in one turn.

E.G: Raw Dmg + HP Cut/Drain

I guess this applies to jutsus like Bloodthirsty Bats as it deals 1 dmg + HP drain.
